# 4. 填写股东信息

为遵守相关法规，Coda 需获取任何在贵公司持股比例达到或超过 25% 的股东（包括自然人或法人实体）信息。

请通过本节上传贵公司的**股东结构说明文件** (**Shareholder structure document)**（可参考[股东结构文件范本](https://drive.google.com/file/d/1XRlyWyBCVE8trRyZNpLAKzcArzP1Z79g/view?usp=sharing)）。

股东结构文件须由公司董事或授权代表签署。

此外，请分别提供所有个人或法人股东的详细信息。您可点击“**添加另一位股东 (Add another Shareholder)**”按钮继续添加。
